This is an obvious question and here is what you need to know:
NicoDerm CQ is a small, nicotine-containing patch. When you put on a NicoDerm CQ patch, nicotine passes through the skin and into your body. NicoDerm CQ is very thin and uses special material to control how fast nicotine passes through the skin. Unlike the sudden jolts of nicotine delivered by cigarettes, the amount of nicotine you receive remains relatively smooth throughout the 24 or 16 hour period you wear the NicoDerm CQ patch. This helps to reduce cravings you may have for nicotine.

Nicoderm Nicotine Patches come in 3 different Steps and must be used in the recommended order (1, 2, & 3).
If you smoke: more than 10 cigarettes per day: start with Step 1.
If you smoke 10 or less cigarettes per day: start with Step 2.

NICODERM CQ PATCH ST1 21MG 14 STEP 1 - 21mg patch
You will start your treatment with Step 1. This step provides you with 14 patches that you need to use in 14 days. You would ideally wear one patch on your arm, chest, shoulder or back for 16 to 24 hours or so. Step 1 patch will send 21 mg of nicotine to your body gradually, over the 24 hours that you have the patch on. This is very different from how you get nicotine from cigarettes, as you get big jolts of nicotine, every time you smoke. So the patch makes your body not get a high doze of nicotine in one shot. This helps disable a high number of nicotine receptors in your brain and starts making you feel better and not crave cigarettes at all.

Nicoderm CQ Step 2 Clear Patch 14mg 14/Box STEP 2 - 14 mg patch
Step 2 contains only 14 mg of nicotine. It works just like Step 1 but has less nicotine. In addition to Step 1, by lowering your nicotine content, this phase will again reduce the number of nicotine receptors in your brain and reduce your cravings even more. Usually by the time you reach this step, you would not feel the need of using patches anymore, however it is still recommended that you finish all 3 steps to make sure you stay off cigarettes. You would continue tihs step for 14 days (2 weeks).

Nicoderm CQ Step 3 Clear Patch 7mg 14/Box STEP 3 - 7 mg patch
Step 3 contains only 7 mg of nicotine. It works just like Step 1 and 2 but has a much lower level of nicotine. This step reduces the number of nicotine receptors to the minimum and prepares you to start living a nicotine free life. You would continue this step for another 2 weeks and that is all. Your treatment would be completed with the completion of the 6th week. Some chain smokers are advised to stay on step 1 for 4 weeks instead of 2 if they feel they are not ready to move to the 2nd step.
